Your nurse carries out home visits, checks your vital signs, reviews your medications, and is your first point of contact for day-to-day health concerns. They know your history and your routine.
Health Coach
Your health coach works with you on the lifestyle side of managing your condition. That means diet guidance, movement recommendations, and practical habits that support your treatment plan.
Care Coordinator
Your care coordinator manages the logistics of your care. They schedule visits, handle communication between your team, update your family after every visit, and make sure nothing falls through the cracks.
